Photo Policy
Level 5 Athletics takes images (still and motion) at many clinics, camps and sessions. Virtual events may be recorded for training and archiving purposes. Unless expressly cited in writing to exclude my child or myself, I understand that the images may be used in Level 5 publications and marketing materials.
When you submit this form, you will be directed to our payment portal. Payments are only refunded if the camp is canceled. No other refunds will be given.
